CLINICAL
NURSING SERVICES
Cancer
Screening for Women: Breast exam and pap smear.
Vouchers are given for mammograms and, if needed, physician follow-up. Cost is based on age and income. Ages 40-64 by appointment.
Immunizations: Walk in clinic Monday through Friday 8:30 am to 4:30 pm. Reduced cost for required
childhood vaccinations if the child is uninsured or under-insured. Pricing for all other vaccinations including
travel vaccines is based on cost of vaccine plus administration charge.
PPD TB Skin
Testing: Walk in clinic Monday
through Wednesday 8:30 am to 4:30 pm.
Cost is price of test plus administration charge. No appointment necessary.
STD/HIV
Testing & Counseling (Sexually Transmitted Diseases/AIDS):
No consent for minors needed. Cost based on type of exam and tests
performed. All information is strictly
confidential. By appointment.
Family Planning: Services include a complete pelvic exam and counseling. Several birth control options are offered:
birth control pills, Depo Provera injections, IUD’s, condoms, foam, and emergency
contraception. Cost is based on family
size and income. By appointment.
Well Child Exams:
Call for information. Cost is
based on family size and income. By
appointment.
Blood
Pressure: Walk in clinic Monday through Friday
8:30 am to 4:30 pm.
(No fee)
Pregnancy Testing:
Walk in clinic Monday through Friday 8:30 am to 4:30 pm. Cost is price
of test plus administration charge.
Baby Your Baby:
Presumptive eligibility to Medicaid for pregnant women. Call for program information and
appointment.
Prenatal Home Visits: Home
visits during and after pregnancy. Women
currently enrolled in Medicaid.
Medicaid Targeted Case Management: Home visits by Registered Nurses. Children 0-3 currently enrolled on Medicaid.
Prenatal to 5 Home Visitation Program: Home visits by certified infant development
Registered Nurses. Referral
required. (No fee).
Morgan Clinic
Site: 48 W.
Young Street, Morgan, Utah 84050. Tuesday 9:00 am to 12:00 noon, services by
appointment. 1:00 to 4:00 pm walk-in

How
do I qualify for WIC? You must live
within Weber or Morgan County and must:
1. Fall
into one of these categories:
ü pregnant woman
ü breastfeeding woman who’s
infant is less than 1 year of age
ü woman with an infant less
than six months of age
ü woman whose pregnancy has
ended less than 6 months ago
ü Infants and children less
than five years of age
2. Be at
nutritional risk
3. Have an earned monthly income (before taxes) not more an the following, based on family size
for the period July 1, 2006 - June 30, 2007

Food Handlers
Testing: Cost $7.00. Testing (English only) offered daily Monday
through Friday 8:30 am to 3:30 pm.
Duplicate card cost $2.00. Spanish class held on the third (3rd)
Tuesday of every month at Ogden-Weber ATC at 2:00 pm. Call ATC to enroll - 395-3700.
On-site food handler and Food Safety Manager testing available. Call for more information.
Wastewater
Certifications: Septic $130.00; Well $35.00.
Contact Environmental Health Office at 399-7175 for wastewater
information packets and fee structure.
Child Care
Home Inspections: $49.00.
Vehicle
Inspection Program: Call 399-7140 for specific information.
Mass Gatherings:
Plan
review $132 for 2 hours + $66 per hour.
Inspections (maximum 3 hours) $66 per hour; $99/hour after hours &
weekends/holidays.
Public
Information: Asbestos,
Radon, Solid/Hazardous Waste, Complaints, Mass Gatherings, Septic/Wastewater
Inspections, Water Sampling, Restaurant and Swimming Pool Inspections.
BIRTH AND DEATH RECORDS
Vital Records:
Birth
Certificates $15.00, Death Certificates $13.00; extra copies issued at the same
time for $8.00 each. All records issued
Monday through Friday 8:30 am to 4:00 pm.
Additional fees for special handling and affidavits may apply.
Community
Health Promotion: Youth and adult smoking
cessation and tobacco education, injury prevention, and bicycle helmet
education.
