VITAL RECORDS - WHAT'S NEW FOR MARCH 2010
NEW STATE FEES: Effective July 1, 2009, the fee schedule changed for services provided by all state and local vital records offices in Utah. Mail orders postmarked after July 1, 2009 will require the new fees.
Birth Certificates, initial copy $18.00
Book Copy Birth Certificates $21.00
Stillbirth Certificates $15.00
Death Certificates, initial copy $16.00
Additional copies of the same record $8.00
Affidavit Fee $7.00
Expedite Fee $12.00
Book Copy Birth Certificates- Book copy records are only for Weber County births from 1958 to 1998 and Ogden City from 1909 to 1998. Book copies sometimes have a little more information on the record than the computer generated birth certificate. They could have the parents occupations or the time of birth, it just depends on the year of birth it is as to what kind of information is on the record. The computer birth certificate or the book copy serve the same purpose and can be used by any agency for identification.

The Weber-Morgan Health Department Office of Vital Records will continue to be open five days a week. However, due to the new four-day work week at the state, we may have limited services on Fridays.
NEW HOURS: We are open until 6 p.m. Monday through Thursday. On Fridays, our hours are 8 to 5 p.m. Requests taken after 5:30 will be processed the following day and on Friday's requests taken after 4:00 p.m. will be processed the next working day.
MORGAN SERVICE NOW BY APPOINTMENT ONLY: As of December 8, 2009, Weber-Morgan's Vital Records office will issue birth and death records at our Morgan County Clinic on the 2nd Tuesday of each month from 12:30 to 3:30 p.m.. To make an appointment, please call (801) 399-7132. We can access records from any county in Utah but the years of the records must be limited to: birth certificates, from 1951 to present, and death certificates, from 2007 to present.
Special services such as amendments and paternities can only be done at Weber-Morgan Health Department in Ogden.
Our Morgan office is located at, 236 E Young Street. We can only accept cash, checks or money orders.
SOCIAL SECURITY NEWS: Parent (s) may inquire directly with the SSA on the status of their child's social security card by calling the Social Security Administration Customer Service Center at 1-1800-772-1213. Parents can also visit the SSA website at Social Security Online for questions such as how long it takes to get a SSA card (in Utah it'sthree weeks from the date of birth), how to get a replacement card, card theft or how to facilitate a name change and what documentswill be needed as proofs. Check it out!
GOODBYE OLD RECORDS: We recently sent dozens of old books with handwritten birth and death records down to the Utah State Archives where they will soon be available online.
It was difficult letting them go because it was fun having them here to look through the old death certificates. The handwritten documents were recorded using quaint language of the day. Some of them noted the cause of death being related to something we don't often see today, such as "a runaway team," worn out," "whiskey drinking," and "dropsy due to a bullet wound from the Civil War." Or, more commonly, "Typhoid Fever," "Diptheria" and "Consumption."
Our older Vital Records will now be safe and cared for properly. If you are looking for one of these records, you can find it at the Utah History Research Center, 300 S. Rio Grande, Salt Lake City, UT 84101. (801) 533-3535 or online at www.historyresearch.utah.gov. We recommend taking a trip down there, the building is as old as some of the records and well preserved.
The Weber-Morgan Health Department continues to maintain birth records, dating from 1909 to present and death records, dating from 1958 to present. We invite those who need copies to drop by our offices and pick one up. Most records can be made available while you wait.
WALLET-SIZE BIRTH CARDS NO LONGER AVAILABLE: As of August 1, 2006, Utah's "wallet size" birth card is no longer accepted for legal purposes. If the card is all you have, it will need to be replaced with a new paper birth certificate. The cost is $18.00. Certified copies issued on rainbow paper prior to August 1, 2006, remain valid. For walk-in customers, we now accept credit and debit cards.
