You can qualify in person by calling for an appointment to see a registered nurse at the Weber-Morgan Health Department, 801-399-7250. Once you apply for Baby Your Baby, because it is temporary coverage, you must apply for Medicaid, which is different from Baby Your Baby. You may apply online with Utah Department of Health | Medicaid or in person at the Department of Workforce Services at 480 27th Street in Ogden.
