Disease Reporting

Utah law requires prompt disease reporting to allow the local health agency to interrupt disease transmission; locate and provide prophylaxis or treatment to exposed contacts; identify and contain outbreaks; ensure effective treatment and follow-up of cases; and alert the medical community.

To report a communicable disease to Weber-Morgan Health Department, please call 801-399-7252.

Disease reporting is available 24 hours a day, 7 days a week. Calls made after-hours or during weekends and holidays will be sent to a confidential voicemail system. This reporting line also allows for emergency calls to be forwarded to an on-call staff member. All messages will receive a prompt response.

The following information is required to be reported with each disease report: [see the Communicable Disease Rule (R386-702) ]

  1. Patient's name, address, phone number, age or date of birth, sex
  2. The diagnosed or laboratory confirmed disease or injury
  3. Date of onset for disease or date injury occurred
  4. Your (person reporting) name and phone number
  5. The laboratory results if available and the laboratory that or is processing clinical specimens for this case
  6. All other information requested by the health department employee taking your report

Relevant laboratory results should accompany a disease report. Disease reports and/or laboratory results should be faxed to 801-399-7153.

Disease Reporting Rule

Diseases are required to be reported according to the Communicable Disease Rule R386-702, under authority of State Statute Sections 26-1-30, 26-6-3, and 26-23-b. All reports required by rule are confidential and are not open to public inspection. Nothing in this rule precludes the discussion of case information with the attending physician or public health workers. Public Health Reporting & HIPAA

Utah law requires reporting certain diseases to the local public health agency and includes providing additional patient information if necessary to assist public health efforts to investigate or respond to disease reports. Patient authorization for these disclosures is waived under federal HIPAA provisions. Report an Animal Bite

Rabies can be transmitted to humans from the saliva of an infected animal, usual through a bite. It is a serious disease that affects the central nervous system, causing disease in the brain and almost always leading to death. The majority of rabies cases reported to the Centers for Disease Control and Prevention each year occur in wild animals like raccoons, skunks, bats, and foxes.

Weber-Morgan Health Department works closely with Weber County Animal Services to monitor and facilitate treatment for individuals who may have been exposed to rabies through an animal bite or contact with a bat.

Report a Foodborne Illness

The Centers for Disease Control and Prevention (CDC) estimates that 1 in 6 Americans gets sick by consuming contaminated foods or beverages each year. These are a common public health problem and often go unreported.

If you think you became sick from something you ate, please let us know. Your information is reviewed by the health department and may help in outbreak investigations. You can report your illness in a few ways:

  1. Submit a report online using IGotSick . This is a web-based reporting system that reviews your symptoms, places you visited and what you ate.
  2. Submit your complaint by phone by calling 801-399-7160. A staff member will collect your information regarding your illness and determine whether further action is necessary.
