Immunization Records Requests

  • Access your immunization records and/or COVID-19 vaccination card electronically on Docket Health. Docket pulls from Utah’s statewide immunization information system (USIIS). Records can be emailed within minutes and at no charge.


  • Complete the AUTHORIZATION TO RELEASE INFORMATION form. Record requests for immunizations may take 4-5 business days. The health department will contact you to let you know they are ready for pickup, and charges may apply.


  • If you don’t have electronic access, you may contact our office at (801) 399-7250 to be put on our records request list.

Childhood, Adult, and Travel Immunizations

Our clinic provides recommended childhood, adult, and travel immunizations.

Immunizations are provided by appointment only. Call (801) 399-7250 to schedule an appointment.

If you have insurance, bring your insurance card to every appointment.

If uninsured, reduced fees may be available for certain services based on income and family/household size. Please bring current proof of income, such as a pay stub within the last 30 days.

Before your appointment, contact your insurance provider to make sure your plan benefits cover your specific immunization and/or service.

Please arrive early to fill out paperwork before your appointment. You may be rescheduled if you are later than 15 minutes.

Please review the vaccine information statement(s) before you or your child receive a vaccination.

Flu Shots

Flu vaccines are recommended annually for all individuals 6 months and older. They are especially important for those at higher risk for complications of influenza including:

  • Children younger than 5 years of age
  • Adults 65 years of age and older
  • Pregnant women
  • Individuals with chronic medical conditions
  • Individuals who live in nursing homes or long term care facilities
  • Individuals who live with or care for people who are high risk for influenza complications

Weber County:

Call 801-399-7250 to schedule an appointment. Come to the Immunization Clinic on the first floor of the Weber-Morgan Health Department main building in downtown Ogden, 477 23rd Street.

Morgan County:

Call 801-399-7250 to schedule an appointment at the Morgan location. Morgan immunization clinics are held at the Morgan City Hall, 90 W. Young Street, on the first Monday and third Friday of every month. Morgan residents may also make an appointment in Ogden.


The COVID-19 Vaccine is now available to anyone over age 6 months. The Weber-Morgan Health Department offers vaccines for all ages by appointment in our immunization clinics.

Lost Card FAQ:

Replacement COVID-19 Vaccination Record

  • Due to increased demand for replacement vaccination records, you must make an appointment to receive a new card ($5) or printout of your entire vaccination record ($2).
  • Depending on the volume of requests we receive, this could take up to 14 days to process.
  • Complete this Authorization to Request Health Information Form.
  • We will respond to your request by email or telephone in the order it was received. We will tell you when to come in and pick up your replacement card. Payments will be made at that time.
  • If you don’t have access to email, call 801-399-7250 and ask to be put on the list to get a replacement card. Be sure to leave your contact information.
