Birth & Death Records
Vital Records staff provides services to the public in obtaining birth, death, marriage, divorce and still birth certificates; affidavits for corrections; voluntary declarations of paternity; hearing screening letters; burial transit permits; and disinterment/re-interment permits.
Food Handler Permits
Individuals that work in facilities that prepare or serve food are required to obtain a Utah Food Handler Permit. Learn more on how to obtain a Utah Food Handler Permit.
Immunizations are provided for individuals of all ages. No walk-ins are available. Please make an appointment.
The food service protection division works to provide education to food service workers and food service businesses in an effort to promote safe food handling and preparation practices.
Health Promotion Classes
Weber-Morgan Health Department Health Promotion & Prevention Services provides classes for diabetes prvention, car seats, and suicide prevention.
The Women, Infant, and Children (WIC) Program teaches low-income families about nutrition and healthy lifestyles.
