Home Visitations

We offer these home visitation services: Nurse Family Partnership, Targeted Case Management, and Prenatal to 5. There is not a cost with these services. The home visits are for pregnant women, infants, and young children by registered nurses. To be part of these services, you will need a referral.

Nurse Family Partnership

A proven public health service that can transform the life of a mother pregnant with her first child. The mothers we serve are connected with a registered nurse early in their pregnancy and receive 30 months of ongoing care that continues through their child's second birthday.

These are the goals of the Nurse Family Partnership (NFP):

  • Improve pregnancy outcomes
    • by helping women engage in good preventative health practices, including obtaining thorough prenatal care from their healthcare providers, improving their diet, and reducing their use tobacco and nicotine products, alcohol and illegal substances.
  • Improve child health and development
    • by helping parents provide responsible and competent care for their children.
  • Improve the economic self-sufficiency of the family
    • by helping parents develop a vision for their own future, plan future pregnancies, continue their education and find employment.

We encourage and depend on health care providers to identify and refer women who qualify for Nurse-Family Partnership services. Eligible participants are women who are currently less than 28 weeks pregnant, will be first-time mothers, and must live in Weber County. Call 801-399-7250 for more information.

Targeted Case Management

Targeted Case Management (TCM) is a service provided in a home visit by a Registered Nurse to assist family and care giver identify and gain access to needed medical, social, educational, and other support services for Medicaid-eligible children. TCM nurses will help Medicaid children access needed services and ensure that services are coordinated among providers and agencies involved.

Testing and Treatment for Minors

Prenatal-5 Home Visitation is a service for pregnant women and children from birth to age five. Home visits are provided by a Registered Nurse to assess health needs, give teaching and parenting information for infants and children who have medical and social risk factors, and coordinate needed services with providers and agencies involved.
