Immigration Physicals

A two-part medical examination required for all individuals applying for admission and citizenship in the United States. A medical examination is one means of evaluating the overall health of immigrants or aliens applying for adjustment of status to a permanent U.S. resident.

We are open from 8:00 AM to 12:00 PM & 1:00 PM to 5:00 PM on Monday thru Friday.

Medical Examination

Two appointments are required to complete an Immigration Physical at Weber Morgan Health Department.


First Appointment:

  • Lab work for Tuberculosis, syphilis, and gonorrhea testing. NO fasting is required for the lab work.
  • Health history information collected.
  • Immunizations required for immigration physicals administered (fees for immunizations are NOT included in the Immigration Physical fee).

Second Appointment:

  • Physical exam with a Civil Surgeon, a licensed doctor who is certified to provide immigration physicals in the United States.
  • Completed I-693 form copied and sealed in envelope. Envelope and additional copy given to patient.

Insurances: We do not have the ability to bill insurances for immigration exams, however some insurances may cover cost of vaccines.

Payment is due at the time of the first appointment and is non-refundable.
