$5-$20 per vaccine for children 0 -18 years of age to qualify depending on household income and family size.

Utah School Immunization Exemption

Effective July 1, 2018

If you would like to obtain a personal or religious immunization exemption for your child, please go to the following website and complete the immunization exemption module:
https://immunize.utah.gov/immunization-education-module/

If you need a medical immunization exemption you will also need to complete the immunization exemption module at the link above and obtain a written notice signed by a licensed healthcare provider.

After completing the immunization exemption module, your immunization exemption certificate will be sent to the email address you provide. You will need to print the certificate and provide it to your child’s school at registration.

Weber County Libraries offer free computer use and printing at the following locations:

Morgan County Library offers free computer use and printing for a small fee.

If you are having problems getting your certificate printed please contact the Weber-Morgan Health Department at
(801) 399-7250 or contact your child’s school.

You can also obtain an immunization exemption for your child at the Weber-Morgan Health Department free of charge. You will need to schedule an appointment by calling (801) 399-7250. At your appointment you will meet with a nurse who will go through the immunization education module with you. Please plan on at least 20-30 minutes for this appointment.

Travel Immunizations

Travel immunizations provided include: yellow fever vaccine, injectable/oral typhoid vaccine, and antimalarial prescriptions. Recommended adult immunizations are also provided. Travel immunizations can be scheduled and given Monday thru Friday by appointment. Limited to residents of Weber and Morgan counties. Please bring a proof of residency (i.e. driver's license).
